• The appointment must not have occurred yet, OR
• The appointment must have occurred within the last 30 days

If the appointment occurred more than 30 days ago, please contact our office before booking this service.

You will be directed to review and sign the service agreement.

Email 1: After signing, you will receive an email with a copy of your signed agreement.

Step 3 – Submit Payment

You will then be directed to complete the payment.

Email 2: After payment is completed, you will receive a payment receipt.

Email 3: You will receive a confirmation email explaining that you will receive access to your secure client portal.

Email 4: You will receive a link to access the Docketwise client portal, where you will complete the intake form and upload your documents.

Once the intake form is submitted, a member of our team will contact you within 24 hours to begin assisting you with your case.
